Registration Information:
Registration for the meeting includes registration for
all scientific sessions, accommodations and all meals and
social events. This includes: dinner Monday evening, the
Monday evening Mixer, Breakfast, Lunch and Dinner Tuesday,
the Tuesday Mixer, and Breakfast and Lunch on Wednesday.
Accommodations are for Monday and Tuesday nights,
either single occupancy (1 bed) or double occupancy (2
beds).

NEW FOR 2008:
This year we were able to reserve additional rooms,
especially more single rooms, which sold out quickly in
2007. HOWEVER, we must commit financially to the extra
rooms by the beginning of 2008, and release the extra rooms
if it appears registrations will not increase over 2007.
PLEASE
REGISTER EARLY!!!! EARLY REGISTRATIONS WILL BE AT A REDUCED
RATE. Payments may be made via PAYPAL, or via check or
money order.
To
qualify for early registration: Paypal payments must
be credited by Dec. 31. 2007. Checks and money orders
must be postmarked by Dec. 31. 2007 (USPS or courier).
(If you wish to register early, but for some reason
cannot make payment by Dec. 31, please contact the
organizers for assistance at the email addresses below).
Registration Procedure:

Complete all fields in the interactive PDF registration
form.

Print
a copy of the PDF for your records and/or save on
your computer.

Email
your completed registration form to BOTH of the
following email addresses:

cmarcus@salud.unm.edu, using “SWP450
REGISTRATION” as the subject line. You should
receive a confirmation within a week indicating receipt and
acceptance of your registration and ROOM ASSIGNMENT; if not,
please contact the organizers at the email addresses above
or by phoning 505-272-3848. (IF we cannot accommodate your
selection of room choice, we will contact you with
alternatives).
